Tribal Card Information/Application and VT State Hunting and Fishing

Please include a self-addressed stamped envelope with a copy of a legal photo that is clear, in color, and with the applicant’s face; the photo must be at least two (2) inches wide as well as a copy of a state issued ID.

***We will need to collect a $10 fee for the processing of applications and issuance of Tribal cards. This money will go directly into Tribal funds for supplies such as ink and card stock, and these funds are non-refundable.

If you are between the age of 15-18 and applying for the first time your $10 fee is waived.

Any cards issued before 2016  should be updated with a new card due to some changes.

If you have had a name change, address change etc. we will need you to update your application.

Starting 1/1/2021 you can apply for a “Vermont State Recognized Tribal Citizen License”. Please visit “Vermont Fish and Wildlife” website for instructions.

The Tribal office volunteers ask that the “certification sheet” that is needed, be filled out to the fullest by you and emailed to [email protected] The Chief will need to sign in the appropriate section. We will then e-mail the signed form back to you. If you do not have email please send your certification form to 100 Grand Ave. Swanton, VT 05488. with a self-stamped addressed envelope so that we can return your certification back to you. (Emailing the certification is preferred and will be faster).
